When to use our Second Opinion Program

1. A new medical diagnosis such as chronic neuropathic pain, complex regional pain syndrome, failed neck/back syndrome, post-laminectomy syndrome, or persistent nerve or neck/back pain.
2. Additional treatment options or opinions on spinal cord neuromodulation, peripheral nerve neuromodulation, or restorative neuromodulation.
3. A change in your medical diagnosis or medical treatment plan.
4. Traveling to Denver is difficult.

How to proceed

1. Please gather your pertinent medical records including imaging prior to your appointment.
2. If a neuromodulation device has been implanted, please gather the device information.
3. Please fill out the new patient application and indicate the need for a 2nd opinion. We will contact you for prompt scheduling and guide you through the process.
